How to Apply

Prospective Students

  1. Complete PharmD application3 in PharmCAS by Dec. 2, 2024. [see PharmD Admissions4 for details on the application process]
    1. Under UCSF-specific essay question "Why UCSF?" -- state your intention to apply to our PharmD-PhD program.
    2. Under UCSF-specific essay question "Optional Information" -- include a Statement of Purpose showing your commitment to clinical education + research.
    3. UCSF requires a minimum of three, and a maximum of four, Letters of Recommendation. One of your recommenders should be familiar with, and address, your research experience. 
  2. Contact Leslie Floren at leslie.floren@ucsf.edu5 to set up a brief meeting to discuss the process.
  3. Complete PSPG application6 by Dec. 1, 2024. [see PSPG Admissions6 for details on the application process]
    1. Include current CV
    2. Include "Comprehensive Research Summary"
    3. Under “Statement of Purpose” section, upload a document that simply states “See UCSF PharmD Application”
    4. Under “Recommendations” section, indicate “See UCSF PharmD Application” in all required boxes. For email, use joel.gonzales@ucsf.edu7
    5. Under "Course History" section, indicate “See UCSF PharmD Application”
    6. Under "Honors, Awards, and Fellowships" section, indicate "See UCSF PharmD Application" in all required boxes. For date, use current month/year

Current UCSF PharmD Students

  1. Complete PSPG application6 by Dec. 1, 2024 [see PSPG Admissions6 for details on the application process]. Include:
    1. Current CV
    2. Comprehensive Research Summary
    3. Statement of Purpose
    4. Three Recommendations (at least one who is familiar with your research experience)
    5. Under "Course History" section, include UCSF Coursework. For pre-UCSF coursework, indicate “See UCSF PharmD Application”
